Sacred Heart provides a variety of recovery support services in withdrawal management, residential and outpatient levels of care. Recovery support is incorporated and focused on care coordination for both emotional and physical wellness such as: transitioning levels of care, supportive and recovery housing, community engagement, case management and peer support. Sacred Heart, housing support services specifically provide a successful transition to the community with housing that is safe, secure, and recovery focused. Our housing program provides a variety of supportive services and welcomes individuals engaged in medication-assisted treatment services. Sacred Heart recovery housing is MARR and NARR accredited.

SUD HEALTH HOME SERVICES
Sacred Heart Substance Use Disorder Health Home (SUD HH) services provide comprehensive care management and coordination services to individuals with substance use disorders. Our health homes elevates the role and importance of peer recovery coaches and nursing services to foster direct empathy and connection to improve overall health and wellness. In doing so, this will attend to an individual's complete health and social needs.
OUTPATIENT SERVICES
Sacred Heart offers a course of outpatient services both in-person and via telehealth. Sacred Heart’s outpatient services also ensure continuation of care for individuals who make sufficient progress in residential treatment to transition to an outpatient level care. These services blend group therapy, didactic group therapy, specialty groups, wellness groups, and individual and family counseling.

RESIDENTIAL INPATIENT SERVICES
Sacred Heart provides clinically managed high-intensity residential 24-hour supportive treatment in a safe, and structured environment to help individuals initiate and/or continue the recovery process. The program’s goal is to teach new skills that are required for a successful transition from active addiction to a stable, recovery-positive lifestyle. Recovery support is incorporated and focused on care coordination for both emotional and physical wellness such as: lower levels of care, housing, community engagement, and peer support. Therapy includes daily groups, lectures and activities designed to meet the needs of clients as defined in their individual treatment plans. Assessments and treatment plan reviews will determine priorities so that an individualized treatment plan can be developed and updated along with individualized notes, amendments, and the individual’s response to therapeutic interventions.
